Important Dates and Reminders:

  • Turn in your collected pledges no later than Friday, May 21, 2010 (including online donations) or have them postmarked by Thursday, May 20, 2010 and if you raise $100+, you may order a free ticket to Swim for Diabetes Night at Progressive Field on Saturday, June 12, 2010. One free ticket per person.
  • The deadline to place your order for Indians tickets is Friday, May 21, 2010. The order form is available at: http://www.diabetespartnership.org/swim/swimfordiabetesnight.asp. Groups wishing to sit together must submit one order for the total number of tickets needed.
  • For those who raised $65 or more, your commemorative Verizon Wireless Swim for Diabetes t-shirt will be available when you pick up your Indians tickets or, if you are not going to the game, your t-shirt will be mailed to you after June 12, 2010.
  • It is not too late to ask additional people to sponsor you or to encourage your sponsors to increase their pledge. Remember, if you raise $300+, you are not only eligible for a free ticket to the June 12, 2010. Indians game, but you also qualify for more great prizes! View a complete list of prizes HERE. The prize you qualify for will be available for you to pick up with your Indians tickets.

Instructions For Collecting Pledges

If you are collecting pledges in person, here are some tips for collecting your pledges easily and safely:

  1. If you are a minor, be sure to bring an adult along with you when asking for and collecting pledges.
  2. Present the certified sponsor sheet to your sponsors and show the amount owed. (Swimmers: You swam 20 lengths and one of your sponsors pledged $.25 per length, your sponsor would owe you $5.00.)
  3. Ask sponsors to make checks payable to Diabetes Partneship of Cleveland or to DAGC.
  4. If sponsor pays with cash, give a receipt (download HERE) and write a personal check in exchange for money given. Cash may get lost in the mail.
  5. Mail your collected pledges and the sponsor sheet in the envelope provided in your Swimmer Packet (or mail to DAGC at 3601 S. Green Rd., #100 Cleveland, OH 44122).

If you have received donations online, make sure to log into your personal fundraising page HERE for an updated total amount of money you have raised online. Money you have raised online counts towards your prizes, but it is your responsibility to keep track of money raised online so you order the appropriate prizes.
