  • Diabetes Resource Guide (228K Acrobat .PDF file)
    This 17-page guide, written for both the general public and for health professionals, includes local hospital and community resources for people with diabetes, a listing of local support groups, and information about Medicare Party B coverage and patient assistance programs.

  • Diabetes Risk Assessments
    DAGC offers FREE diabetes risk assessments (including blood sugar testing) every Tuesday morning from 9:30-11am.

  • Community Education Programs
    DAGC offers 6-8 FREE community education programs annually at various locations throughout northeast Ohio.
