Prize and Indians Ticket Order Forms

The deadline to complete these forms was Monday, May 18, 2009. If you have any questions, please call our office at

Ticket/Prize* pickup:

Tuesday, June 9th and Wednesday, June 10th:
-8:30 am - 6:00 pm at the DAGC office at 3601 S. Green Rd., #100, Beachwood

Wednesday, June 17th:
-From 5:30 -7:00 pm at Progressive Field at the SWIM/SCUBA for Diabetes table in front of Gate C (East 9th Street & Eagle Avenue).
-After 7:00 PM, tickets will be held at Will Call. Any prizes not picked up by 7pm will be available at the DAGC office.

*If you are not going to the game and you qualified for and ordered a prize, your prize will be mailed to you.
