Community Partnerships and Collaborations

We are committed to providing personalized long term support for clients and assisting them in navigating the health care system through collaborations with other organizations, such as the Cuyahoga County Division of Children and Family Services and Adult and Senior Services, Centers for Dialysis Care, Ohio Hispanic Social Worker Association, and Garfield Heights Senior Center.  In addition we have connected with the Social Work Departments and Care Coordinators at all of the major hospital systems in the Greater Cleveland area. 

At present, our diabetes educators are out in the community providing diabetes self-management and support programs to low-income populations at the Hunger Network, Care Alliance Health Center, Cleveland Metropolitan Housing Authority and Euclid Library, to name a few.

Our staff has represented Diabetes Partnership on a number of community initiatives including the Health Improvement Partnership Cuyahoga, Better Health Partnership, Healthy Cleveland Initiative, Cuyahoga County Health Alliance, Cleveland Food Bank, Safety Net Providers Strategic Alliance of Clinicians, Northeast Ohio Association for Hispanic Health, and the Prevention Research Center at Case Western Reserve University.
