Prize and Indians Ticket Order Forms

Ticket/T-Shirt*/Prize pickup:

Tuesday, June 9th and Wednesday, June 10th:
-8:30 am - 6:00 pm at the DAGC office at 3601 S. Green Rd., #100, Beachwood

Wednesday, June 17th:
-From 5:30 -7:00 pm at Progressive Field at the SWIM for Diabetes table in front of Gate C (East 9th Street & Eagle Avenue).
-After 7:00 PM, tickets will be held at Will Call. Any t-shirt and prizes not picked up by 7pm will be available at the DAGC office.

*If you are not going to the game, your t-shirt will be mailed to you after June 1, 2009.
